          MEDICAL SOCIAL WORKER

          Position Summary:The essential functions of this position include, but are not limited to, providing psychosocial evaluations and treatment under the direction of a physician's plan of treatment to patients who are receiving home health/hospice services.

          Qualifications:

          * Masters degree from school of social work accredited by Council on Social Work Education.
          * Two years experience in a social agency or community organization dealing with health and/or welfare.
          * Registration in the state of Michigan as a Certified Social Worker.
          * Current Michigan driver's license and access to reliable transportation.
          * Ability to obtain CPR certification prior to making solo visits.
          * Service Area: All service areas serviced by MidMichigan Home Care
