JOB SUMMARY

Provides medically necessary diabetes nutritional outpatient self-management training to patients as ordered by the physician. Represents the program to physicians, local and national organizations, and community groups. Identifies and utilizes available and appropriate community services to enhance the program. Ensures complete documentation entries are made in outpatient charts and communication of education forms and notes to the referral physicians. Participates in data collection and analysis to meet the program evaluation system objectives.

REQUIRED QULAIFICATIONS

* Bachelor’s Degree in Nutrition or Dietetics
* Certified Diabetes Educator or certified within 6 months of hire/transfer
PREFERRED QUALIFICATIONS

* Registered dietitian with a minimum of 1-3 years clinical nutrition experience
